• Uniforms: Players must wear uniforms for every practice.

• No Refunds: There is no refund if you sign up and decide not to play. No refund is provided for leagues or tournaments.

• Tuition: Tuition is collected every month.

• Missed Practices: There is no reduction in tuition for missed practices.

• Holiday Practices: There will be no practice when a holiday falls on a practice date.

• Method of Payment: The monthly fee can be paid by check or cash. If payment is made by credit card or credit, a 3.9% processing fee will be charged. If you have automatic payments, there will be no charge.

• Late Payments: A $25 fee will be applied for late payments. Payments are expected to be made between the 1st and the 5th of each month.

• NSF: A $25 fee will be charged for any insufficient funds (NSF).

• Termination: If you wish to discontinue soccer practices, you must provide a two-week written notice or email cerritosacademy@yahoo.com.

• Winter Session Commitment: There is a minimum four-month commitment during the winter session.

• Unacceptable Behavior: Cerritos Soccer Academy reserves the right to terminate practices for unacceptable behavior.

• Risk Assumption: I further understand that sports involve physical contact between players and that serious accidents occasionally occur, resulting in personal injuries (including death) and property damage. Knowing the risks of participation, I now agree to assume those risks and release and hold harmless Cerritos Soccer Academy, coaches, agents, and employees who might otherwise be liable for damages.

• Use of Media: As a parent or guardian of this student, I hereby consent to the use of photographs/videotape taken during soccer training for publicity, promotional, and/or educational purposes (including publications, presentations, or broadcasts via newspaper, internet, or other media sources). I do this with full knowledge and consent and waive all claims for compensation or damages.

Refund Request Information

To request a refund for registration, please submit your request in writing via email to cerritossocceracademy@yahoo.com or via text to 240-553-3264 or 240-673-3426. Written requests are required for documentation purposes.

Refund Guidelines:

• Processing Fee: All refund requests are subject to a $30 processing fee.

• Timing of Request: Refund requests must be submitted before the program activity begins. Requests made by telephone or after the first team or program activity will not be honored.

• Refund Processing Time: Refund requests will be processed within one to two weeks from the date the request is received. If the refund is approved, a check or an electronic credit will be issued to the credit card originally charged. An email confirmation will be sent once the refund is processed.

• Uniform Fees: Any player who has been issued a uniform or had a jersey imprinted by their coach will be charged an additional uniform fee.

• Automatic Payments: If you are enrolled in automatic payments, please notify us at least one week in advance if you wish to cancel. Refunds will not be applied for cancellations made after this timeframe.

• Injury-Related Refunds: To apply for an injury-related refund, the player must submit a doctor’s report that includes details such as the condition of the player, the probable duration of the injury, and any recommended physical therapy. The player should also indicate whether they intend to remain on the roster after recovering from the injury and completing any necessary rehabilitation.
